Posting Summary | Rutgers, The Statue University of New Jersey, is seeking a Community Outreach Worker (CHW) for NJMS-Medicine-Infectious Disease. The primary purpose of the Community Outreach Worker position is to connect and engage to care clients either lost to follow-up or who have never accessed care. Serves as a role model and community liaison, and work as part of the medical case management team to address barriers to connection and retention in care. Supports providers and the case management team through an integrated approach to care management and community outreach. Provides reliable information, appropriate referrals, and emotional support to those clients who have agreed to participate in the program. Helps clients to access services and resources and address issues of adherence through individual and group sessions. Among the key duties of this position are the following:
