Posting Summary
Rutgers, The State University of New Jersey, is seeking a Patient Navigator for NJMS- Emergency Medicine. The Patient Navigator supports various aspects of the Navigator community project funded by CDC. The Patient Navigator will engage in Community outreach, activities and events for those in need of opioid use disorder treatment.

Among the key duties of this position are the following:
  • Conducts community outreach activities to connect individuals with care and services, including opioid treatment programs and medication-assisted treatment providers
  • Provides guidance and support to individuals seeking assistance with opioid use disorder
  • Collaborates with hospital and community partners to coordinate medical care and social services for individuals referred from the Emergency Department (ED) or other community programs.
  • Respond alongside the Community Paramedic to overdose calls in the community, providing immediate support and assistance to individuals experiencing opioid-related emergencies.

Qualifications

Minimum Education and Experience
  • Bachelor's Degree in social work or Human Services
  • Plus a minimum of two (2) years of fulltime professional experience working with patients diagnosed with OUD or SUD.
  • Equivalent education, experience, and/or training may be substituted for the degree requirement.
Certifications/Licenses
Required Knowledge, Skills, and Abilities
  • Ability to speak clearly and write proficiently.
  • Hearing is essential.
  • Must be able to focus on the task at hand despite numerous interruptions.
  • Must be able to sit for an extended period and document into an electronic medical record.

Physical Demands and Work Environment

  • The position requires working in a moderate noise office setting. Fast-paced, high-pressure office environment; must be able to cooperate with a broad spectrum of staff and site visits in the University Hospital's Emergency and Observation Department.
